UK HSE: load falls from lorry and kills cyclist
A UK Health and Safety Executive (HSE) investigation revealed that a Warwick haulier was fined £90,000.
An unsecured 2.5-ton heat exchanger fell from a lorry on a bend in Nuneaton. The load shifted, snapping a strap and tragically killing a passing cyclist.
An HSE investigation into the fatal incident determined critical safety failures:
Poor Security:
The heat exchangers were inadequately secured and highly prone to toppling.
Damaged Equipment:
The ratchet straps used to secure the load were in poor condition.
Lack of Training:
The driver was never provided with a securing scheme by his employer, nor had he received training for transporting unusual, high-risk loads.
